Complete the form below to withdraw from classes.

We are a continuous program with reoccurring monthly tuition. You will incur reoccurring monthly tuition charges until you submit a Mills Gymnastics USA Stop Class form. If you are stopping a class, it must be done on or before the last day of the month. If you stop a class after the month begins you will not receive credits and/or refunds for the remaining classes in the current month.

Below is an electronic version of our withdrawal form and can be submitted to stop classes. The Stop Class Form can also be printed, click here for a printable version, fill out and submit via email, or simply stop by our front desk.

Please allow 2 business days for this request to be processed. This form must be submitted no later than the 27th of the month with a minimum of 2 business days to be processed for the next month.

If you are having a scheduling conflict, we will be happy to help you switch to another day and time or address any other issues you may be have with your current class.

I understand that submitting this form to Mills Gymnastics USA on or before the last day of the month goes into effect for the following month. Remaining classes will be completed for the current month.
